Useful Techniques for Complete Excision of Large Conjunctival Cysts by a Transconjunctival Approach

Abstract
Purpose: Conjunctival cysts can occur spontaneously, after ocular trauma or surgery, or after chronic conjunctival disorders. Complete excision is usually recommended to prevent the recurrence. In this report, we describe our experiences of complete excision of large conjunctival cysts by transconjunctival approach.

Methods: A retrospective review of all patients who underwent excision of conjunctival cysts, larger than 9 mm in the longest dimension, using hydrodissection and en bloc resection of fibrous band with conjunctiva between 2006 and 2012 at Chungnam National University Hospital was performed.

Results: Six patients with large conjunctival cysts were treated by surgical excision. Similar to hydrodissection technique in cataract surgery, injection of 1% dental lidocaine beneath conjunctiva dissected effectively the cyst wall from conjunctiva. All cysts were excised completely, but 3 of them were ruptured during surgery. The cyst was ruptured during the attempt to dissect of fibrous bands on conjunctival cyst. In the other 3 cases, en bloc resection of fibrous bands with the conjunctiva was done, and it prevented the rupture of cyst. There were no sign of recurrence in all cases.

Conclusions: The transconjunctival approach is an effective method to remove a large conjunctival cyst completely. Hydrodissection and en bloc resection of fibrous band with conjunctiva are useful techniques during the surgery.
